Transfer of Medical Records Form

To ensure we have a complete understanding of your medical history, new patients are asked to fill in a medical records transfer form to authorise the transfer of your records from your previous doctor or medical practice. This helps us provide you with safe, consistent and personalised care.

Please note this is still required if you previously saw Dr Kumar at another practice, as records are held by the practice, not the individual doctor.

As this form requires a physical signature, it must be completed on paper rather than an online form.

Please print the PDF and fill out with pen. Completed forms can either be scanned then emailed to [email protected] OR can be dropped into reception (8 Nilmar Avenue Wodonga).

Alternatively if you do not have a printer and/or scanner, our reception staff will be happy to provide you with a copy to fill out.
