We offer two types of Health Assessment: one for patients aged 75 and over, and one for patients aged 45-49.

We also offer a Health Heart Check for patients aged 45 and over (or 30 and over if you’re of Aboriginal or Torres Strait Islander descent).

Health Assessments for Age 75 and Over

At LifeBridge Medical Centre, we offer comprehensive annual health assessments for patients aged 75 and older. These assessments help identify, manage, and prevent health issues, ensuring you maintain your independence and quality of life.

What’s Included in the Assessment?

  • Functional Health Evaluation: Reviewing daily living activities like cooking, dressing, and mobility.
  • Preventive Care: Discussion of vaccinations, health screenings, and nutritional advice.

How to Book Your Annual Health Assessment

If you or a loved one is aged 75 or older, it’s time to prioritize your health.

Aged 45 to 49?

If you are aged 45-49, you may be eligible for a one-off FREE comprehensive health assessment with one of our nurses and your usual doctor.

A 45-49 Health Assessment involves our nurse  interviewing you, reviewing your recent medical and family history, and checking your blood pressure and other health metrics to determine your risk of developing a chronic condition such as heart disease, diabetes or cancer.

You will then have an appointment with your usual GP, who can then discuss your individual health needs, consider additional medical tests and offer advice on how to lower your risk factors so that you can improve your overall health, prevent illness and live longer and with better quality.

The full assessment takes approximately one hour, and is fully Medicare funded.
